The deputy coroner has recommended police continually review the way they handle people with mental illness.
The recommendation comes after an inquest into the police shooting of a Melbourne man in 2004.
Jason Chapman of Yarraville was shot three times by police after threatening them with a knife outside his home.
The deputy state coroner, Ian Treloar West, found police put themselves and the public at risk by failing to cordon off the area before the shooting.
He said doing so would have given police more room to retreat and could have allowed them to calm the situation.
Mr Chapman suffered from schizophrenia and the deputy coroner said police procedures for dealing with people with mental disorders should be developed with help from mental health professionals.
Mr Chapman's family learned of his death from a television news report and the deputy coroner also recommended police review their procedures for informing next of kin of a death.
